New to SDP?

Build your path into Self-Determination

We help participants and families understand the steps, organize information, and move from goals to an implementable plan.

A practical transition roadmap

1. Understand where you are

Review orientation status, current IPP, current services, recent expenditures, goals, and questions about the transition.

2. Person-Centered Planning

Identify what is important to and for the participant, strengths, communication preferences, relationships, routines, hopes, and support needs.

3. Individual Budget

Organize the information used in budget development and prepare to discuss current services, unmet needs, and relevant changes.

4. Spending Plan

Map approved funds to services and supports that help achieve IPP goals and objectives.

5. FMS & Providers

Understand FMS setup and coordinate selected providers, support workers, community resources, and onboarding steps.

6. Implement

Track authorizations, agreements, provider starts, outstanding actions, and the participant's transition into SDP.

The participant remains the decision-maker. Independent Facilitation is designed to support informed choices, planning, coordination, and advocacy—not to make decisions for the participant.
