Help the participant prepare, participate, communicate priorities, document follow-up, and advocate during person-centered planning and IPP development.
Ongoing Independent Facilitation
Implementation, coordination, budget monitoring, service changes, problem-solving, and annual renewal based on the participant's selected scope.
